The Foster Care Placement Coordinator is responsible for working with Adams County caseworkers on family preservation efforts and appropriately placing youth in Foster Homes when youth are not able to remain with family.

The Placement Coordinator serves as liaison to Child Placement Agencies, QRTPs and Adams County Foster Homes as needed. i.e. communicate acceptance or deferral reasons for foster youth placement referrals as immediately as possible, inquire further about referrals where information is unclear or missing to help in the matching process, and respond to additional placement questions from referral sources as needed.

Primary Job Duties Include:

  • Coordinate appropriate placements of youth in Foster Homes and based on an assessment of the history of the youth referred, that best meet the youth's needs and placement criteria and overseen by Placement Supervisor for final approval.
  • Coordinate the intake process with assigned caseworker for youth being placed in the program.
  • Request, secure, & collect special circumstances documents from required parties as needed prior to placement.
  • Develop and maintain relationships with counties in order to increase positive relationships with Child Placement Agencies and QRTPs and facilitate better matching of foster youth.

Some evening hours may be required to perform specific tasks including securing same-day placement emergencies.
